Activated Collectibles

Cup of Joe		"It feels attached to you"	Grants a random cup effect. Isaac is unable to collect active items while holding it.
Death Pen		"Get rid of them!"	Kills a random enemy.
Encyclopedia of Diseases		"Sickening Guideline"	Applies a random status effect on all enemies. While held, improves status effects in general.
Sales Catalogue		"Portable shop"	Spawns a random shop pickup in the floor.

Passive Collectibles

Blood Flies		"Breeding host"	Upgrades blue flies to blood flies. Brings enemy blood to Isaac that can be converted into health.
Bloodstone		"LUB-DUB"	Prevents damage upon Isaac's heart beat.
Bone Key		"Be the warden"	Defeating the main boss of the floor grants free access to most rooms.
Bubblebath Bonbons		"Unhealthy"	Red hearts are replaced by pink bonbons. Guarantees different effects for every ten bonbons collected.
Clockwork Virus		"Organic metal"	Tears become stationary and spin when Isaac stops shooting. Spinning tears deal damage per tick.
Conjured Knife		"Cursed enemies"	Enemies can be cursed with improved abilities and stats. Increases damage by defeating them.
Creepy Canteen		"Bite them!"	Creeps occasionaly bite and stun enemies. Doubles creep damage and duration.
Critical Tomatoes		"Splash!"	Chance to fire tomato tears that deal splash damage to enemies.
Demonship		"Loyalty brings power"	Increases damage per pact. Devil rooms have free items, but you can only take one.
Eternal Flame		"Candle tears"	Chance to fire candle tears that shoots an eternal beam in the direction that it moves.
Eyerot		"Power demands equality"	Tears distribute damage to all enemies of the same type.
Infectious Lactation		"Yummy!"	Chance to fire milk tears that inflict a temporary milk effect on enemies. Enemies leave slippery creep and receive x1.5 damage from tears.
Ink Stain		"Spread the pain"	Tears temporarily give enemies a black tint (covering them with ink). Tinted enemies receive synchronized damage.
Jaded Ring		"Mental shield"	Speed down and immunity to status effects.
Keratitis		"Phasing Tears"	Tears pierce through enemy armor.
Panacea		"You feel better"	Checks if current stats are lower than character initial stats, which then covert them to initial stats.
Possessive Mask		"Dual personality"	Follows Isaac around while changing between two faces that alter its attack form to comedy or tragedy.
Sheaf of Papers		"Plagiarism"	Chance to duplicate a random enemy upon entering a room. Defeating the enemy grants a random pickup.
Skin Wyrm		"Strength at a price"	Increases damage and converts all soul hearts into black hearts upon pickup. Decreases Isaac's immunity frames by half.
Startling Statue		"Teleports around"	Teleports around while staying stationary.
Suspicious Painting		"I see you!"	Eyes appear from the ground, shooting Isaac's tears on nearby enemies.
Telekill Alloy		"Registered trademark"	Prevents homing projectiles and lasers from detecting Isaac. Upon taking damage, destroys all enemy projectiles onscreen.
Tickle Monster		"Little fella"	Follows Isaac around. Tries to hug enemies, applying charm effect or turning them friendly.
Toothbrush		"Perfect cleaning"	Increases tear damage and delay. Tears that damage enemies can leave tear creep in the ground.
Wendigo Skull		"Flesh eater"	Enemies can drop flesh that can be used to increase damage. Tries to resist damage, losing temporarily damage bonus and requiring repair with bones.
